ThriveDX will allow Company’s employees (“Eligible Employees”) and Family Members (the “Family Members”) to enroll in programs developed and offered by ThriveDX as determined by the Parties under the terms of this Agreement.
To qualify for the benefits outlined in the Education Service Agreement, individuals must be 16 years of age or older. Additionally, any person between the ages of 16 and 18 must provide written parental consent to enroll in the Program.
ThriveDX commits to extending the advantages, including the tuition discount, to family members of Company’s employees. Family Members are defined as: spouse, biological child, stepchild, legally adopted child, child for whom the employee is a court-appointed guardian, and the child of a domestic partner. For the purpose of this agreement, family members also include a domestic partner engaged in a committed long-term relationship, whether with a partner of the same or opposite sex, provided neither party is legally married to other individuals.
